COTA & PT

Per Diem position for Trumbull, Mahoning and Columbiana counties.  This candidate should possess at least one year experience with outstanding patient skills. SOCIAL WORKER

Full Time position is available to provide home care/hospice care services in Trumbull, Mahoning and Columbiana County.  Must have a Masters Degree and one year of social work in a health care setting.  This position will be responsible for initial assessment of patient needs, developing plan of care and providing necessary counseling.  Must have excellent communication and written skills.
