Who can make referrals?
If you or a loved one require care in your home as the result of an illness or injury, contact mvi HomeCare. It's that simple.
We will work with your physician to determine the medically necessary home health care services you may need. We will then develop a plan of care that will allow us to help you reach the recovery goals established by your physician.
If you or a loved one are a patient in a hospital or skilled facility, you may require home health care services at the time of discharge. Remember, that you have the right to choose your home health care provider.
Hospitals are required to inform you of all of the home health care providers available in your area. We encourage you to choose mvi HomeCare. One phone call is all it takes. Call our office at 330-759-9487 or 1-800-449-4mvi. We will do the rest. We will contact your physician and determine your need for home health care services as well as verify your benefit information with your insurance company.
As a free standing, independently owned home health care company, mvi HomeCare can tailor our services to meet your specific needs and the specific needs of your patients. We can customize our delivery of services, frequency of care and frequency of communication for you.
Our Hospital Coordinator will come to the hospital, your office, skilled facility or the patient's home to assess a patient for eligibility for Home HealthCare Services. Remember, you and your patients have the right to choose your home health care provider. If you refer a patient that does not meet home care criteria, we will inform you of their status and assist you in accessing other community services that they may need.
Case Managers may also make referrals directly to mvi HomeCare. Our full range of services, expansive service area and two regional offices make us an attractive alternative for case managers working to meet the needs of their clients.
