RN Case Manager

mvi HomeCare is hiring for multiple RN (Registered Nurse) Positions: Join our team and enjoy the benefits of autonomy while providing one-on-one care to your patients.

You will need:

  • Strong interpersonal skills to build personal rapport and establish trust while providing high-quality care with the goal of increasing patients’ independence and comfort.

  • Willingness to work efficiently and independently

  • Flexibility and patience to work in a variety of living environments.

  • Clinical skills including changing bandages, giving tube feedings, administering medication, and setting up and monitoring IVs.

 Required Experience: 1 year of relevant RN acute care. Educational Requirements: Graduate of accredited School of Registered Nursing. Current license to practice as a Registered Nurse in Ohio. Valid driver’s license.

Weekday On-Call RN


mvi HomeCare is seeking an exceptional nurse for our Weekday

On-Call position for Mahoning and Trumbull Counties.

On-Call hours from Monday – Friday 5pm to 8am (this is a 32-hour position). Health insurance, PTO, and 401k.

Home Care experience preferred.

Duties and responsibilities include but are not limited to evaluate and treat patients, participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives, collaborate with an interdisciplinary team to provide highest level of care, assists RN team Member with assessment and plan of care.

Intake Support Specialist


mvi HomeCare is looking for an LPN to serve as an Intake Support Specialist in their Youngstown office. Hours are Monday-Friday between 8am-6pm. The ideal candidate will be responsible for:

  • Coordination of Home Care/Hospice referrals

  • Collect needed patient medical information and complete the New Patient Referral Form

  • Accept patients based upon meeting appropriate agency and payor eligibility requirements

  • Coordinate the discharge of patients from hospitals, rehab facilities and physician offices with internal staff and scheduling department

  • Assign Case Manager to patients on a regional basis, when possible

  • Notify pharmacy of all IV referrals

  • Knowledgeable and compliant with Medicare and Medicaid Conditions of Participation

  • Knowledgeable with managed care contracts

  • Assist UR with planning and coordinating of home care/hospice services for patients referred to the agency for care.
